Patient forms
Our office requires the "New Patient Registration Forms" to be completed for your first visit to our office.
Please print and complete these forms to bring with you to to your appointment. Forms can also be completed online as they are PDF fillable, saved as a document and emailed back to our office at medrecs@lincolnsurgicalgroup.com. They will not automatically drop/load into our electronic health records. Mobile insurance cards can also be emailed to the same email address.
If you do not have forms completed upon arrival, there may be a delay in seeing the doctor or your appointment may have to be rescheduled.
